The Participating Provider Option plan gives you freedom of choice, flexibility, and a broad range of benefit options. With the PPO product, in many cases there’s no requirement to have your care coordinated through a primary care physician. This means seeking a referral to see a specialist or to get a second opinion could become a thing of the past. You may have the ability to choose doctors and hospitals nationwide.
The Health Maintenance Organization plan gives members access to a broad range of benefits when care is provided by or coordinated through a primary care physician. Female members can also choose a Woman's Principal Health Care Provider.
Traditional Health Insurance
You can apply for a Traditional Health Insurance policy at any time. You (and any eligible dependents you want added to your policy) must meet medical underwriting guidelines, you will have to complete a medical application that may include medical questions, and you may be required to undergo a physical examination to be considered for coverage. Coverage is available through national and local carriers such as BlueCross BlueShield, Aetna, GHI, HIP, Golden Rule, John Alden, Humana, PacifiCare, UniCare, Harvard Pilgrim, Celtic, and Tufts HealthPlan. Policy options, premiums, and benefits vary by carrier and location.
Guaranteed Issue Health Insurance
Guaranteed Issue Health Insurance is available in most locations. The cost of individual and family coverage is based on a group rate by age class. All participants in an age class pay the same amount, regardless of where they live or their medical history. You do not have to submit medical evidence of insurability, and you cannot be turned down for enrollment regardless of pre-existing conditions. You must enroll within the enrollment period.
Note: This Guaranteed Issue Health Insurance is not available in Wisconsin and Hawaii.
